ORDERING  PRESCRIPTIONS

1. Repeat Prescriptions

If you are on a regular prescription we can normally arrange for you to receive this without seeing a doctor. Any medications that you are allowed to receive in this way are listed on the right side of your prescription on a tear off slip . You can obtain a repeat  prescription by the following methods :

• On-line  - 
  The form presented should be completed and submitted. Please tick the "Repeat" Checkbox on the form.
  
  
• Using the tear off slip from your last prescription put a tick at the medication(s)  you require. You can return this form to the surgery by post or fax (0844 477 8973) or in person.

 2.  Acute Prescriptions

If the item you require is not on your repeat medication tick list it will be classed as an acute item. These are medications which are generally not appropriate to put on your repeat list and require closer monitoring by your GP. You can reorder any items in this category by the following methods :

• On-Line -
  The form presented should be completed and submitted. Please tick the "Acute" Checkbox on the form.
  
  
• In person at the surgery were you will be asked to complete an appropriate form.
