REGISTER AS A NEW PATIENT

GENERAL

Our practice list is close to full . In order to provide a satisfactory quality of service to our existing patients we are only accepting applications from patients who are moving into our practice area and who are not currently registered with another doctor in the area.

To  register permanently you require to be planning to be resident in the practice area for minimum of 3 months. If you are staying for less than 3 months then you may be entitled to register as a temporary patient.

Please note that  registrations are only are only accepted between  09:00- 17:30 (Mon - Fri)

Patients who have previously been registered with a GP in the UK

If you would like to register with us , either pick up the following forms from reception or click on the links below . If you are downloading please complete the forms and return them in person to the surgery for registration

b) Medical Questionnaire

Patients from EEA countries registering with a GP in the UK for the first time.

If you would like to register with us , either pick up the following forms from reception or click on the links below. If you are downloading please complete the forms and return them in person to the surgery for registration. You will require to show your passport or identity card along with the appropriate documents as detailed in the application form  . The receptionist will take a copy of these documents

a) GPR
b) Medical Questionnaire
c) Application to register with NHS from EEA Nationals

Patients from Non-EEA Countries registering with a GP in the UK for the first time

Entitlement to register with a GP in the UK will be dependant on having the required proof of entitlement. You will require to show your passport/visa .In addition :

If you Visa has been issued for study you will require to show  a copy of the university/college acceptance letter for the course you will be /are attending.

If your Visa has been issued for employment you will require to show a current letter from your employer detailing your role , employment status and hours of duty and/or a copy of your contact of employment.

If your Visa has been issued as a Work or Study dependent  we may require to see your marriage certificate or other identification.

If you have entered the UK and are seeking asylum  we wil require to see a copy of Home Office authority or correspondence relating to your application

If your Visa has been issued for holiday, travel etc you are not normally entitled to register with the NHS. If your country has a reciprocal health agreement with the UK you may be entitled to be seen by an NHS GP but registration will normally be on temporary basis.

Applications to register should be made in person to the Medical Centre. The receptionist will take a copy of the various documents supporting your application 

 Change of Address

If you  change your address please CLICK  here to download a change of address form. This should be completed and forwarded to us as soon as possible so that we can update our records. If you move outwith the practice boundary you must  register with another doctor who covers your new address
