What are Community Link Workers?

A Community Link Worker (CLW) is a social practitioner who is part of the primary care team based at your GP practice. A CLW is not a nurse or a doctor but part of the primary care team who work closely with your GP and can help you to improve your mental and emotional wellbeing. They can support you to access wellbeing activities and services.


Your CLW can:
• Spend time with you to talk about what really matters to you
• Help you to identify any problems or issues you are experiencing
• Support you to set goals and work with you to achieve these

They will keep up to date with what is happening locally so they can tell you about new services and resources and they can go with you to visit these services.

Everything that you discuss with your CLW is confidential (unless we believe there is a risk of harm to yourself or others).

Who is my CLW?

Springwell Medical Centre welcomes Kirsten Marwick to the practice. Kirsten will be with us on Tuesdays and Wednesdays. Please let your GP or any member of the practice know if you would like a referral.

For a referral, please speak to any member of staff in the practice.
